Direct answer
Acid reflux, or GERD, is stomach acid washing into the food pipe. It causes heartburn, a sour taste, or cough, often worse after meals or when lying down. Most people improve with meal timing, weight loss and prescribed acid medicine. Surgery, including fundoplication for a hiatal hernia, is considered when symptoms continue on proper treatment, or when a large hernia causes food to stick or come back up. Dr Neel Patel assesses this at Niraaya Hospital, Kudasan, Gandhinagar.
Key points
- Night-time burning, sour fluid in the throat, and chronic cough can all be reflux.
- A hiatal hernia means part of the stomach has slid up through the diaphragm.
- Medicines are the usual starting point. They do not fix a large mechanical hernia.
- Difficulty swallowing, bleeding, or weight loss needs prompt assessment, not another antacid.
What does acid reflux feel like?
Heartburn is a burning rise behind the breastbone. Some people feel food or sour liquid coming back into the throat, a chronic cough, hoarseness, or the need to clear the throat at night. Symptoms often follow a large meal, tea, coffee, spicy food, or lying down soon after eating.
A hiatal hernia is a separate structural problem that often travels with reflux. Part of the stomach slides into the chest through the opening in the diaphragm. A small hernia may cause nothing. A larger one can cause pressure, early fullness, or food that will not go down.
When are medicines enough?
For most adults in Gandhinagar and Ahmedabad, the first plan is practical and medical:
- Finish dinner two to three hours before lying down
- Raise the head of the bed if night symptoms dominate
- Reduce weight if it is above target, and cut back on late tea, fizzy drinks and heavy fried meals
- Take the acid-suppressing medicine your doctor prescribes, at the right time, for a full trial
Stopping medicine early and then calling it a failure is a common reason people think they need surgery too soon. A specialist review checks whether the dose, timing and diagnosis were right.
Symptoms that should not be managed at home
- Food sticking in the chest
- Unintentional weight loss
- Vomiting blood or black stools
- Chest pain that could be cardiac. Rule the heart out first
- Anaemia or persistent vomiting
When is hiatal hernia or reflux surgery considered?
Fundoplication is an operation that wraps the upper stomach around the lower food pipe to strengthen the valve. It is considered when:
- Reflux is proven and still troublesome after a proper medicine trial
- You cannot stay on long-term medicine, or you choose surgery after counselling
- A large hiatal hernia is causing mechanical symptoms
Before surgery, Dr Neel Patel may advise endoscopy and, when needed, tests that measure acid exposure and the way the food pipe squeezes. Operating without that picture leads to poor results. The procedure is usually laparoscopic. More on evaluation is on our stomach and oesophageal disorders page.
Life after a successful repair
Patients start with soft meals and small portions. Some feel full quickly or cannot belch easily for a few weeks. These effects usually ease. The aim is fewer night symptoms and less dependence on daily acid tablets, not a licence to ignore meal size forever.
Frequently asked questions
Does every hiatal hernia need surgery?
No. Small hernias with controlled reflux are often watched or treated with medicine. Surgery is for persistent symptoms or a hernia that is causing mechanical trouble.
Can I stay on acidity tablets for years?
Many people do, safely, under medical advice. If you still have symptoms, need rising doses, or want to stop tablets, ask whether tests and surgery are a better plan.
Is fundoplication done in Gandhinagar?
Selected patients can have laparoscopic fundoplication with Dr Neel Patel at Niraaya Hospital, Kudasan, after the right tests.
Is milk or cold water a treatment for reflux?
They may soothe for a few minutes. They do not heal an inflamed food pipe or repair a hernia. Use them only as comfort while you arrange a proper review.
